Medical Record Management
Handling a patient’s medical record is outlined by Resolution 1995 of 1999 issued by the Ministry of Health of Colombia and by Judgment T-650/99 of the Constitutional Court.
The medical record is a document that originates from the relationship between the Clinic and the patient. It contains all the follow-up that specialized doctors have maintained with the patient since the first appointment; therefore, it is the only legally accepted document by both parties.
The medical record is of a private nature. It is understood that it is only relevant to the person being treated and the physicians involved in the process. That is why there are special conditions for the release of this information:
- If you personally request your medical record, you only need to present your identification document to the staff at the Admissions desk.
- If a third party will be requesting your medical record, you must send written authorization along with your identification document. The third party must also present their identification document.
- If the patient is a minor, only the father or mother can request the medical record, and they must attach the minor’s identification document.
- If the medical record of a deceased person is requested, it will be delivered after obtaining prior authorization issued by the judicial entities. Likewise, if it is a first-degree relative, they must provide the death certificate and a document certifying the family relationship (marriage certificate, birth certificate).
- In the case of a request by a judicial entity, the medical record of a patient will be delivered upon presentation of a written request issued by the competent authority and with the approval of the institution.
- Clínica del Campestre and the patient or their representative may send the medical record by email to the prepaid medical care entities and insurance companies that request this support to authorize surgeries and other procedures.
- To receive your medical record by email, please fill out the authorization form. The record will be sent within 24 hours after receiving the completed authorization form along with a copy of the patient’s identification document.
- If you are in another city or country, you can receive your medical record by email by sending the request and your corresponding identification document.
If you meet the requirements mentioned in the previous cases, our Archive staff will search the system to print and deliver the medical record.
